This Word template lays out a case report in the order the CARE guideline expects, from patient information to follow-up, with a table for the clinical timeline.

What is inside

  • Title, keywords, structured abstract
  • Introduction, patient information, clinical findings, timeline table
  • Diagnostic assessment, therapeutic intervention, follow-up and outcomes
  • Discussion, patient perspective, informed-consent statement, declarations

How to use it

  1. Remove every patient identifier before drafting; consent for publication must be documented.
  2. Fill the timeline table first; it keeps the narrative accurate.
  3. Download the official CARE checklist and tick it against your finished draft.
  4. Check your target journal's case report format; many limit length and sections.

Example (illustrative)

A timeline row might read: Day 0 — presented with 3 days of fever and rash; Day 2 — blood cultures taken. (Illustrative example, not a real patient.)

Limitations and cautions

It structures the manuscript but does not make a case publishable. Consent for publication, de-identification and your institution's case-report policy come first, and journals set their own length and section limits. The file is free to use and edit, and the footer credits MedResearch Ninja.
